Forms list and editor improvements

Forms list: Each row has Preview, Duplicate, and Edit (with icons). On smaller screens the action labels hide and you get icon buttons instead, then the actions stack so the list is easier to scan.

Form editor:

  • Label, Help text, and Placeholder show examples that match the field type
  • New Phone field for telephone numbers, with a phone-style keypad on mobile
  • If something is missing when you save, the error appears next to the field and at the bottom of the page
